We appreciate partnering with you and have given you access to our online scheduling function. Typically, our practice screens people for appropriate fit and makes sure they are seeing the right therapist. Also, we screen to make sure we are in network with their insurance. We trust that you have reviewed our information and have chosen the best candidate. We ask the following of you:

  1. Make the appointment through our secure website using the client’s phone number and email.
  2. Send us a copy of their demographic information, or face sheet.
  3. Insurance information if it is not available on the face sheet.
  4. Please send us a discharge summary or explanation for the referral prior to their appointment.
  5. Sliding scale options are available and should be discussed over the phone or via email ahead of time.
  6. Please provide the client with the following PDF (Referral
    Form)    .

NOTE: Our facility is not handicap accessible and does require the ability to walk up a flight of stairs.
